    Note that by filling in this questionnaire you are authorizing Kalpresh Properties LLC and its staff to use this information in your initial screening process towards your tenancy. You are promising Kalpresh Properties that you will provide correct and truthful information within the questionnaire. This initial screening doesn’t guarantee any housing. You understand that a separate application will have to be processed with RHR that will involve a complete investigation may include any or all of the following: Credit Report, Criminal Record, Rental History References (including MPHA), Employment Verification, Eviction Records and Personal Interviews with references. This authorization is for this transaction only and continues for (1) year unless limited by state law, in which case the authorization continues in effect for the maximum period allowed by law.This period will automatically be extended if a lease agreement is signed between the applicant and Kalpresh Properties. Kalpresh Properties will have the right to terminate the lease agreement in future if any of the supplied information is found incorrect at a later time during the lease. Kalpresh Properties reserves the right to conduct or not conduct a showing. Incomplete applications will not be responded to. We believe in giving equal opportunity to all prospects. By submitting requested information, Prospect Tenant(s) understands and agrees to (a) all the risks as well as conditions within this document (b) the FAQ (Proposed Lease), Rent Affordability and Questionnaire documents which must be read before submitting Application. Kalpresh Properties LLC reserves the right to produce this document in the court of law or may use this during any disputes or argument with Prospect Tenant(s) in future.
